Your LAC MK Retreat Registration is not complete until you complete the following forms:
- Medical Treatment Authorization – You must download, complete, and send this form with your child to the retreat.
- Authorization for Foreign Travel with a Minor – These are templates to be used if possible in your country that will allow your minor to travel with only one parent or no parents internationally. Ultimately, you are responsible to adapt these forms to whatever your country and airline require. If what we have is not able to be used in your country, you will be required to find a minor travel document either online, through a lawyer, or with the airline that will work for you and your circumstances to allow your minor to travel.
